DIRECT BILLING IS AVAILABLE AT FORM HEALTH
Registered massage therapy is frequently covered by extended health benefit plans. RMTs can request payment be made directly to them as a convenience to the client.
Check with your insurance company to see if a doctor's note prescribing massage is required in order to claim coverage for this service.
The direct billing is simple and fast. Your health care practitioner (RMT) submits your insurance claim on your behalf. The insurance company may pay the provider (your RMT) directly, but some insurance plans may require the insured to pay up front. If this is the case with your plan, your RMT can submit your claim and request that the reimbursement be sent directly to you.
